About this document
An official bilingual veterinary certificate form for the non-commercial entry into the EU of up to five dogs, cats or ferrets from third countries, based on EU Regulation 998/2003. The certificate is signed by an official veterinarian of the country of departure and is required when entering Italy or any EU country with a pet. Part I holds the owner and animal data (microchip or tattoo, date of birth); Part II is the veterinary attestation: rabies vaccination at least 21 days before travel, an antibody test and a waiting period for certain routes, and echinococcus treatment for dogs. The regulation has been updated over time - check the current form and requirements before travelling.

How to use it
Use the form as a sample to understand the document structure and gather the data in advance: microchip number, vaccination dates, vaccine batch. It is filled in and signed by an official veterinarian of the departure country shortly before the trip. Verify the current version of the form and the rules for your route - requirements have changed. The document can be viewed page by page in the protected viewer.

Key information
- Part I: information about the sender/recipient, country of origin, animal description, identification number (microchip or tattoo), date of birth.
- Part II: veterinary inspection: the animal meets the definition of "non-productive" (Regulation (EC) No 998/2003, Art. 3(a)); has undergone primary rabies vaccination at least 21 days prior; revaccination within the validity of the previous one.
- Alternative requirement for countries from Annex II to Reg. 998/2003: when transiting through a country NOT from Annex II - virus neutralization test (>=0.5 IU/ml) no earlier than 30 days after vaccination + 3-month waiting period.
- Table II.4: vaccination details (microchip/tattoo number, vaccination date, product, batch number, validity, blood sampling date).
- II.5/II.6: for dogs - treatment against Echinococcus multilocularis according to Delegated Regulation (EU) No 1152/2011, Art. 7 (details in Table II.6).
