Official autocertificazione form (autocertificazione) based on DPR 28.12.2000 n.445, confirming stable affective relationships between two persons. Used to confirm that a foreign citizen will spend the mandatory quarantine period and the remainder of their stay in Italy at the declarant's address.

An official self-declaration blank (autocertificazione) based on DPR 445/2000 confirming a stable affective relationship between two people. It was widely used during COVID restrictions: the foreign partner declared they would spend the mandatory quarantine and the rest of the stay at the declarant address in Italy. The form is also used as proof of de facto partnership without formal marriage - for example, in family reunification cases or to support coesione familiare. It records the declarant details, the partner details (name, date and place of birth, residence) and the shared address. False statements carry criminal liability under art. 76 DPR 445/2000.

How to use it
Fill in both partners details and the shared address, add place, date and signature, and attach a copy of the declarant ID. Remember: the self-declaration is only one piece of evidence of the relationship; authorities usually ask for other proof of cohabitation too. Check current requirements. Why you need it
Used (especially during COVID restrictions) to document the fact of cohabitation of a couple without official status (no marriage) upon entry into Italy. Also used when applying for family reunification or to confirm partnership.
