How to use it

The host in Italy fills in the form, ticks the undertakings actually accepted, signs it and passes it to the guest with a copy of their ID (and the fideiussione if stated). The guest attaches everything to the visa application. Check your specific consulate requirements. The document can be viewed page by page in the protected viewer.

Why you need it

Mandatory document in the visa package when a foreigner enters to stay with a private individual or organization: confirms accommodation, financial responsibility, insurance. Data is processed in the VIS (Visa Information System) and stored for up to 5 years.

Key information

• Inviter's data: name, surname, date and place of birth, nationality, identity document/passport/permesso di soggiorno, address, profession, phone/email.
• For organizations: ragione sociale, sede legale, position and name of the representative.
• Invitee's data: name, surname, date and place of birth, nationality, passport, address, profession, nature of relationship (familiare/family member), purpose and duration of visit.
• Inviter's declarations (checkboxes):
• - Ready to host at the specified address;
• - Takes responsibility for expenses during the stay;
• - Has arranged medical insurance in the guest's name;
• - Provided fidejussione bancaria in the amount of [amount] euros at the bank [details].
• The host confirms awareness of the duty to notify the public security authority of the foreign national's presence in their home within 48 hours of entry into Italy (art. 7 D.Lgs. 286/1998).
• The host confirms awareness of the criminal liability under art. 12 D.Lgs. 286/1998.
• The data is kept in VIS for up to 5 years; under certain conditions it is available to EUROPOL (prevention of terrorism and serious crime); the data controller is the Ministero degli Affari Esteri e della Cooperazione Internazionale.
• Annexes (Allegati): a copy of the host's ID, the fideiussione bancaria, other documents.
• VIS data is available to the visa authorities, external border checks, the authorities checking entry and stay, and asylum authorities of the Schengen states; in Italy, under certain conditions, also the Ministero dell'Interno and the police.
• You may obtain your VIS data in any Schengen state, have inaccurate data corrected and unlawfully processed data deleted; the Italian MFA is at Piazzale della Farnesina 1, 00135 Roma, the supervisory authority is the Garante per la Protezione dei Dati Personali (Piazza di Montecitorio 121, Roma).
