A universal self-declaration blank - dichiarazione sostitutiva di certificazione under art. 46 DPR 445/2000. It lets you confirm facts about yourself (civil status, residence, kinship and so on) without notarization and without collecting official certificates. It is one of the most used documents in Italian bureaucracy: at the questura, the comune, in permesso applications, SSN registration and tax matters. The signature needs no certification (art. 74 DPR 445/2000), and an authority refusing to accept a self-declaration where the law allows it breaches official duties. False statements carry criminal liability and loss of the benefit obtained (art. 75-76 DPR 445/2000).

How to use it
Enter your details, state the fact being confirmed in the free text field, add place, date and signature, and attach a copy of your ID. Why you need it
One of the most commonly used documents in Italian bureaucracy. Used in Questura, Comune, when applying for permesso, when registering for SSN, when filing tax returns, etc. Signature does not require notarization (art. 74 DPR 445/2000).
