The official Dichiarazione di ospitalita o di garanzia form (fac-simile) - a declaration by a host in Italy for a foreigner applying for a tourist visa. The host - an Italian citizen or a foreigner with a residence permit - confirms the guest accommodation, covers living and medical costs and guarantees the guest departure when the visa expires. A two-page document with explanatory footnotes: declarant details with codice fiscale, ID type (document, passport or permesso with questura data), guest details, stay period, housing status and size, annual income and its source. The signature needs no notarization if given before an officer or with a copy of a valid ID attached (art. 38 D.P.R. 445/2000).
