How to use it

Fill in Modulo 2 together with Modulo 1 as part of the kit permesso: block letters, black pen, only the sections relevant to your employment type. Income amounts must match your declarations (UNICO, CUD, busta paga). Practice with our kit-filler before filing. The document can be viewed page by page in the protected viewer.

Why you need it

Attached to Modulo 1 when submitted to Questura or through Sportello Unico Immigrazione (post office). Contains data on the type of work activity, employer, contract, income, and registration with Centri per l'Impiego.

Key information

• Section 1: type of employment - lavoro subordinato (employment), lavoro autonomo (self-employment), other.
• Section 2: employer data for employment: name, codice fiscale / Partita IVA, matricola INPS, name of the individual employer, number of their permesso.
• Section 3: contratto di soggiorno - signed or submitted to Sportello Unico Immigrazione; date and province of signing.
• Section 4: data for lavoro autonomo: name, codice fiscale, Partita IVA, matricola INPS, province and registration number in C.C.I.A.A., registration date, albi/registri of professional activity.
• Section 5: income - type of declaration (Modello UNICO, CUD, busta paga, bollettino INPS), amount in euros (annual or monthly), year of last declaration submission; separate field for activities started in the current year.
• Section 6: registration in the anagrafico Centro per l'Impiego list (formerly 'liste di collocamento'): date, province, city, circoscrizione.
• The form is three pages long, all fields must be filled in capital letters with a black pen.
