The official form of the Italian Ministry of the Interior MOD. 209 MODULO 1 - the main application form for issuance, renewal, update, duplication or conversion of a permesso di soggiorno or carta di soggiorno. It is completed when applying for any type of permesso via the kit postale at a Poste Italiane office. Eight sections cover the full data set: request type and permesso typology code, list of completed modules and number of dependent children, personal data with codice fiscale and refugee status, passport data, visa and entry data (date, border point, visa number and type), travel document request, address in Italy with contacts and an alternative mailing address. This is the clean 8-page blank without the code tables; a scan of the whole envelope with the tables (country codes, grounds, provinces) and Modulo 2 is a separate document in the library.

How to use it
This is the first module of the kit permesso: fill it in block letters with a black pen, take country and typology codes from the kit instructions. Working applicants add Modulo 2. A wrong visa number or entry date is a common reason for returns. Practice in the kit-filler. The document can be viewed page by page in the protected viewer.

- Section 1 - Request data: surname, name, province/Comune of domicile, type of request (rilascio/rinnovo/aggiornamento/duplicato/conversione), type of document (permesso / carta di soggiorno), code of permesso type, number of existing permesso, expiration date.
- Section 2 - Data on the application: which modules are filled out (Modulo 1/2), total number of pages, number of dependent children.
- Section 3 - Personal data: codice fiscale, marital status, gender, date of birth, country of birth code, citizenship code, refugee status, city of birth.
- Section 4 - ID document data: passport or other document, number, expiration date, issuing authority.
- Section 5 - Visa data: date of entry into Italy, border point, visa number, visa type, single or multiple entry, annotations/motives for the visa, validity period.
- Section 6 - Request for renewal of title: type of foreign travel document/stateless/refugee, period (1 or 2 years).
- Section 7 - Contact in Italy: province, Comune, address, house number, scale, internal, CAP, email, landline, mobile.
- Section 8 - Contact for communications: alternative address for correspondence (if different).
