- Housing (one of the options): purchase/rental agreement; declaration under Art. 46-47 DPR 445/2000; declaration from an Italian citizen or legally residing foreigner providing housing
- Income for the previous financial year: above the minimum for exemption from medical expenses (8,263.31 EUR; if spouse present - up to 11,362.05 EUR; plus 516.46 EUR for each dependent)
- Marca da bollo: 16 EUR
- Submission methods: in person by applicant (if in Italy); applicant + authorized delegate (specified at submission); attorney (procuratore), if applicant is abroad - procura translated and legalized
